Supporting Data-Driven Screening and Prevention Services using OSCAR
Working with CPC Clinical Research and the Las Animas-Huerfano Counties District Health Department (LAHCDHD), One Health Insights has supported screening and prevention services using OSCAR (Outreach, Screening and Referral System) to connect over 1,000 participants to healthcare and community resources. This program screens individuals for a variety of health risk factors and provides a referral pathway for appropriate services based on screening results.
Beyond direct program support, OHI has partnered with LAHCDHD to build internal capacity and ensure long-term sustainability of these efforts. This has included comprehensive staff training on the effective and consistent use of the OSCAR platform, equipping team members with the skills needed to conduct screenings, manage referrals, and engage participants in a meaningful way. OHI has also supported the development and implementation of best-practice policies and procedures to promote data integrity, participant confidentiality, and operational efficiency across the program.
In addition, OHI has led efforts to design and implement a standardized data infrastructure, including processes for securely storing, transforming, and maintaining program data. This structured approach has enabled the development of dynamic data dashboards that provide real-time insights into screening activities, referral outcomes, and population reach. These tools allow stakeholders to monitor trends, assess whether priority populations are being effectively served, and identify gaps or emerging needs within the community.
Ultimately, this data-driven approach supports ongoing evaluation and continuous quality improvement, ensuring that services remain responsive, effective, and aligned with community health priorities.
